Job Title: Community Health Extension Workers

- Our mission as community health extension worker is to deliver superior community and home-based healthcare services, prioritizing prevention, promotion, and basic curative care within designated clusters.
- We also stand ready to provide prompt support for REACH clinic activities, ensuring comprehensive and timely care for all individuals in need.
Duties And Responsibilities
Purpose:
- Act as healthcare educators, providing vital education about the healthcare options that are available.
- Ensure cultural competence among healthcare providers serving vulnerable populations.
- Inform EHA and educate other stakeholders about community health needs. Build community capacity to address health issues.
- Meet with clients, individuals, and community organizations to create health programs designed for individual use and for groups. Help patients navigate healthcare and social service systems.
- Manage care and care transitions for vulnerable populations. Reduce social isolation among patients. Advocate for access to health benefits and resources for less privileged patients or vulnerable populations within their communities.
Engagement:
- Participate in outreach relative to community awareness of general health, new diets/exercise programs and health risks to targeted groups of people within the community.
- Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition.
Delivery:
- Ensure all necessary notes, forms and supplementary information through weekly checks and updates are well maintained in clients’ database to promote efficiency.
- Complete administrative tasks on an ongoing basis to ensure proper and organized record keeping, working with staff members when/if necessary.
- Determine individuals’ eligibility and enroll them in health insurance plans. Supervise the treatment of patients living with terminal illness so that they can live healthier and longer lives.
Targeted Specialized Knowledge
- Certificate of training as a community health worker (CHEW)Â
- Valid practice license from the Community Health Practitioners Registration Board of NigeriaÂ
- Minimum of 1 years relevant experience
- Fluent in any local language and proficiency in written and spoken English.
